Please follow the steps below for direct access to the evaluation for the Women's Cardiometabolic Health and Wellness Masterclass

IMPORTANT FIRST STEP

In order to access this evaluation form, you must first REGISTER or login on this website. Please note this is a separate site from www.cardiometabolichealth.org. To begin registration steps:

1. Click on the Register/Take Course tab on this page and follow the instructions.
2. Once the above step is complete you will be able to access the evaluation. Please note this website will refer to this as a "Course", please disregard this. Once you are registered, you will have direct access to the evaluation survey to claim your credits. 

Evaluations must be completed by Friday, September 30, 2022. Upon completion of the CME/CE evaluation, attendees can download their general sessions certificate.

Receiving Separate Satellite Symposia Certificates
For satellite symposia, certificates will be provided separately via email within 5-7 business days after the Sept. 30 deadline.

For Pharmacists: Pharmacists have until September 30 to complete the evaluation and claim credit for participation so that information can be submitted to CPE Monitor as required. Expect credit uploads to be completed within 5-7 business days after the evaluation deadline.
