Successful Stroke Rehabilitation: Meet the Collaborative Care Team

This presentation will provide a detailed description of the components of a collaborative stroke rehabilitation team and the characteristics which lead to successful integration focused on quality patient outcomes. Case studies will be used to describe each team member’s expertise and how each individual team member’s expertise is integrated into specific patient goals for rehabilitation. Topics including medical management, rehabilitation therapy, psychosocial issues and community resources will be discussed in the context of how a successful team communicates and coordinates rehabilitation care for stroke survivors.
